To 5urd and iAdam1n: You added new firmware versions to the list and left the modem version empty. In the current format this means it has the same modem version as the iOS above and not that it's unknown. Please verify all your previous edits here that you are sure about the modem version you added (or silently added by refering to above). We don't want incorrect data here.
