This is a factory firmware based on iPhone OS 1.1.2. It appears to be a newer version of Alpine 1A420, featuring a newer version of SkankPhone.app with the built-in Operator screen.
This firmware was originally spotted on a 2007 iPhone PVT but has since been seen on multiple devices.
